CO-03 Inbound & onward referrals

  • Ability to accept and manage multiple inbound referrals (from a variety of systems) with the ability to refer onwards within the organisation to multiple services (as required per referral).

Core Level Requirements

Description

Community care relies heavily on inbound and onward referrals with patients' treatment being long-term and requiring input from primary care settings and other secondary care services. These referrals need to be recorded and actionable and manageable with minimum effort.

Then referrals from multiple sources can be accepted and managed

Description

AF195 Review Patient Referral

The ability of a receiving service/clinician to review new referrals that have been made which they can accept, reject or request further information. Where a referral is rejected, or further information is required the sender must be notified and be able to provide additional supporting information in a resubmission of the referral.

The ability of the system to display a view of referrals made on behalf of a patient and the outcome of those referrals.

As a clinical professional,

I want to be able to refer or re-refer a patient onwards, to the most appropriate care setting or clinical speciality across Organisational and Agency boundaries, ensuring that there is full visibility within the care record of the patient journey.

So that a patient can referred to a different care setting, in a different organisation or agency

Acceptance criteria 1:

Given a user is referring a patient that needs alternative care,

When the user chooses options for referral,

Then a patient can be referred onwards to a different care setting in a different organisation or agency

So that an inbound patient requiring care can be allocated to a caseload and care will be planned accordingly

Given a need to receive referrals from other care settings and another organisation is referring a patient that needs alternative care,

When the user chooses options for referrals received inbound,

Then a patient can be referred inbound to the appropriate care setting, from a different organisation or agency
