Recording of bio-psycho-social multi-disciplinary co-produced (with patients) care plans, including crisis, relapse and contingency plans, and a suite of Mental Health related measures/ratings consistent with National Standards of professional records and Clinical Standards for Mental Health care

Description
Care plans comprise a key area of treatment in the Mental Health setting and EPR systems are required to show the details of these plans and share them with the required care providers and patients.

As a Mental Health Professional, I want to record multi-disciplinary care plans, to include crisis, relapse and contingency, So that, the history of the patient is available to all disciplines. Acceptance criteria 1:Given a user has the correct permissions, When planning patient care that includes input from other disciplines, Then the care plan should be accessible by other disciplines, And the care plan should include information on crisis, relapse and contingency where required. | |

Acceptance criteria 1:Given that a user has the correct permissions, When a care plan is being created, Then the patient can input into the care plan either directly or via a clinical user. And the care plan should be accessible by other disciplines, And the care plan should include information on crisis, relapse and contingency where required. | |
