Description

Care plans comprise a key area of treatment in the Mental Health setting and EPR systems are required to show the details of these plans and share them with the required care providers and patients.

MH-02.1 - Multi-Disciplinary Care Planning

DCF Core Capability

As a Mental Health Professional,

I want to record multi-disciplinary care plans, to include crisis, relapse and contingency,

So that, the history of the patient is available to all disciplines.

Acceptance criteria 1:

Given a user has the correct permissions,

When planning patient care that includes input from other disciplines,

Then the care plan should be accessible by other disciplines,

And the care plan should include information on crisis, relapse and contingency where required.

AF316 Cross Organisational Workflow

Review of care plan by clinicians across multiple organisations

AF317 Patient access to records

The ability of an authenticated patient to be able to view their own medical record including:

  • All relevant documents, e.g. discharge summary, care plans

  • All relevant part of care record.

  • Ability to specify contact preferences

Note: It must be possible for clinicians to be able to restrict information for sharing with the patient via the portal where necessary.

AF341 Digital Presentation of Handover Data

The system provides a digital presentation of patient data including context as part of a digital handover to enable the sharing of relevant patient data without the need for paper-based handover notes

MH-02.2 - Patient Coproduction

DCF Core Capability

As a Mental Health Professional,

I want to be able to add to multi-disciplinary care plans as well as co-production with the patient,

So that the patient's concerns and ideas are registered.

Acceptance criteria 1:

Given that a user has the correct permissions,

When a care plan is being created,

Then the patient can input into the care plan either directly or via a clinical user.

And the care plan should be accessible by other disciplines,

And the care plan should include information on crisis, relapse and contingency where required.
