For a Patient:

For Community Pharmacy Clinical Services:

    • The sharing of information about the service with the Authority as part of service monitoring and evaluation.

    • The sharing of information about the service with the NHSBSA and the Authority for the purpose of contract management and as part of Post-Payment Verification (PPV).

    • Apart from the Pharmacy Contraception Service, Consultation Information will also be shared with the Patient's registered GP Practice.   • For Pharmacy First Urgent Medicine Supply Consultation Information the Quantity Supplied of Schedule 3, 4 and 5 Controlled Drugs listed in Pharmacy First Urgent Medicine Supply Schedule 3, 4 and 5 Controlled Drugs 5 Days Maximum Quantity Data and any related Actual Medicinal Products (AMPs) or Actual Medicinal Product Packs (AMPPs), to not exceed the Maximum Quantity

  • Prevent the recording of the following Controlled Drugs using the Pharmacy First Urgent Medicine Supply Service, as their pack size exceeds the Maximum Quantity for 5 days supply:
