Description
Enables Citizens and Patients that use health monitoring Solutions to share monitoring data with health and care professionals to support remote delivery of care and increase self-care outside of clinical settings.
Outcomes
For a Patient: | Using medical devices and monitoring equipment at home or on the move to regularly record and share vital health information with clinicians to receive support and assistance. |
For General Practice: | Real time remote monitoring of patients to assist in diagnosis, support and decision making for prevention and anticipatory care. |
For the wider NHS and social care system: | Supporting and facilitating the provision of health and social care in non-clinical settings to best support the needs of all Patients regardless of their location. |
