Background
When a document is hidden from a Patient’s Electronic Patient Record (EPR); either provisionally or permanently, all associated read coded information (e.g. SNOMED codes) remains visible, unless manually made invisible. This is considered a potential clinical risk as some coded information that accompanies a referral is potentially harmful.
There are currently 1.9 million documents filed as “hidden” but with clinical codes attached. These will not be exposed automatically by the accelerated access change, however, the risk is present if either new documents are added or for Patient’s that require retrospective access.
The model is being updated with clarification added to the E00174 requirement in Patient Information Maintenance - GP Capability.
